Impact of Obesity and Expression of Obesity-Related Genes in the Progression of Prostate Cancer in African American Men

Purpose of the StudyTo identify the association between obesity and the biochemical recurrence of prostatecancer; association between the expression of obesity-related genes and the biochemicalrecurrence of prostate cancer after initial surgery among obese African American men andnon-obese African American men. MethodsThe primary outcome of interest was biochemical recurrence of prostate cancer (BCR). Therewere 48 African American prostate cancer patients in the study. We collected 127 tissuesamples [normal, prostatic intraepithelial neoplasia (PIN), and tumor] from patients. Weassembled 99 obesity-related genes and determined the levels of their expression with theuse of nCounter gene expression assays by Nanostring Technologies. Trend test of geneexpression among tumor, PIN and normal tissues was conducted. Tests for associationbetween obesity and BCR; and gene expression and BCR were conducted using logisticregression models at a 95% confidence interval respectively. Patients were followed upfrom the date of first surgery to the date of biochemical recurrence or date of lastfollow-up and a Kaplan Meier curve was generated depicting the time to event (biochemicalrecurrence) among obese and non-obese African American prostate cancer patients.Results43 obesity-related genes were significantly associated with biochemical recurrence. Theassociation between obesity and BCR showed a non-significant two-fold increased risk ofbiochemical recurrence in obese African American men compared to non-obese AfricanAmerican men, (OR= 2.032, 95% CI = 0.220 - 18.765, p-value= 0.5318). 20 genes showed anupward trend in gene expression among normal, PIN and tumor tissue samples includingALOX12, ALOX15, CRYBB2, EIF5A, ERG, GNPDA2, HNF1B, HSD3B1, KLK4. ConclusionOur study did not find an association between obesity and biochemical recurrence ofprostate cancer. However, it elucidated some obesity-related genes that could explainprostate cancer carcinogenesis.
